This form documents what occurred to trigger a formal referral and what the supervisor hopes to achieve through this referral.

Supervisory/Management Referral Release of Information Form - Download
The supervisor and employee should jointly complete and sign what information will be released back to the supervisor. A copy of this form should be given the employee at the time of their signature.
